- This program is provided by Caribbean American Life Assurance Company, part of Assurant, Inc.
- To enroll, you must have female reproductive organs, be between the ages of 18 and 64 years old and live in Puerto Rico.
- Your coverage ends:
- if the monlthy payment is not received
- after the 31-day grace period has expired and you have not yet paid your premium;
- in the event of your death
- upon turning the age of 69

How do I get the routing number of my financial institution?
You can find your bank's routing number in the lower-left corner of a check. You can also find it by accessing your bank's website. It is usually displayed on the homepage.
